The theme of our third annual conference is Achieving Integrated Care Through Healthcare Navigation.
Many Canadians face complex and fragmented systems when seeking and participating in care for their health concerns. Integrated care is crucially important in providing a coordinated approach to care planning, facilitating coordination across care providers, and providing client-centered support that incorporates family and caregivers in the care plan. Integrated care can be achieved through quality healthcare navigation.
CHNC 2023 will provide a forum to share and discuss how navigation can support the integration of care through the lens of research, practice, education, and administration.
Conference presentations may touch on innovative collaborations and partnerships between navigation programs and their local communities, how navigators can advocate for their clients to ensure the integration and coordination of care plans, the role of navigation in integrated care systems and mechanisms, and outcomes of projects and initiatives evaluating the role of navigation in integrating care for various target populations. By driving integrated care, healthcare navigation has the potential to mitigate inequalities in access, outcomes, and experiences; enhance system efficiencies; and improve health and healthcare outcomes for Canadians.
